PROCESSAMENTO DE SINAIS DE IMPEDÂNCIA TORÁCICA: ESTUDO PRELIMINAR NA DOENÇA CORONARIANA

Resumo

Simultaneamente com a realização da prova de esforço para avaliação da função ventricular, foram feitas medições-contínuas do sinal de impedância torácica com o objetivo de verificar a sua potencialidade no diagnostico não-invasivo de doença arterial coronariana. 28 pacientes foram estudados e classificados em quatro grupos de acordo com o laudo do eletrocardiograma (ECG) e da cineangiocoronariografia. O sinal da impedância (Z) e o ECG foram gravaqos em fita magnética e processados em um computador digital. Apos o cálculo da media coerente de Z, sua primeira derivada (dZ/dt) foi calculada por metodos numéricos e filtrada digitalmente. Em pacientes com obstruções coronarianas o sinal dZ/dt apresenta um padrão deformado no início da sístole que não e observado em indivíduos normais. Esta diferença está presente em repouso mas desaparece com o esforço submáximo, retornand porem após 10 minutos de interrupção do exercício. Sugere-se que este padrão deformado esteja refletindo uma contração ventricular normal durante a fase isovolumétrica e que esta distorção seja corrigida pelo aumento da contratilidade miocárdica que acompanha o exercício.

Abstract

Thoracic impedance measurements have been carried out during exercise stress testing for evaluation of ventricular function, with the objective of investigating its potentiality for the noninvasive diagnosis of coronary artery disease (CAD). 28 patients have been studied and classified into four groups according to the result of coronary arteriography and the exercise eletrocardiogram (ECG). The impedance signal (Z) and the ECG have been continuously recorded on magnetic tape during the test. The first derivative, dZ/dt have been computed by numerical methods from the coherent average of the Z signals selected according to their heart rate. In patients with CAD the dZ/dt waveform presents an early systolic distortion which cannot be identified in normal subjects. This abnormal pattern is present at rest, but disappears with the exercise, returning however, 10 minutes after recovery from submaximal effort. It is suggested that the distorted systolic wave reflects an abnormal pattern of three-dimensional contraction during the isovolumetric phase which is corrected by improvements in myocardial contractility induced by exercise.
