RADIOLOGÍA DIAGNÓSTICA Y PROGRAMA DE GARANTÍA DE CAUDAD: EVALUACIÓN CRíTICA

Resumo

En el Servicio de Radiología dei Hospital de la Policia Militar dei Estado de Río de Janeiro, se llevó a cabo un Programa de Garantía de Calidad (PGC), con el objetivo de maximizar la obtención de imágenes de buena calidad diagnóstica y con esto elevar el nivel de desempeno en dicho departamento. Las metas fijadas en tal sentido fueron: mejorar la calificación de los profesionales, la atención ai paciente y la calidad de la imagen; optimizar la dosis por examen y reducir los costos. La metodología seguida abordó algunas medidas de carácter gerencial como motivación y aJopción de una filosofia de trabajo; establecimiento de los procedimientos de análisis de las radiografias perdidas, de inspección , y de administración de calidad; el análisis de las imágenes obtenidas antes y después de la aplicación dei PGC y el establecimiento de sus ventajas económicas en radiología diagnóstica. Entre los resultados obtenidos, sobresale la reducción de 70% en la tasa dei rechazo de placas junto con una efectiva mejoría de la calidad de imagen. Se verificó, también, una reducción de los costos de las pérdidas de alrededor dei 75%. Por no dar continuidad ai PGC, diecinueve meses después, fueron constatados aumentos en la tasa dei rechazo de placas y en los costos, retomando a la situación anterior. Estos resultados no dejan dudas en cuanto a la necesidad de tales programas y evidencian que ellos deben ser continuos y contar con el apoyo total de las autoridades responsables, no solamente por los aspectos económicos, sino sobre todo, sociales. Abstract

Quality Assurance Program (QAP) was implemented at the Radiology Service of the Military Police Hospital of the State of Rio de Janeiro (HPM), with the objective to establish a methodology which maximizes the probability of obtaining good diagnostic quality images, alIowing to reach a high performance leveI of the radiology service. In order to reach this objective, some administrative goals were fixed, such as improving patient's care and professionals qualification, and creat a working environment based on sharing responsability andsolidary participation; and inspection goals, such as obtaining and maintaining high quality standards in each step of the radiographic processo AlI this aiming improvement of the image quality, optimizing dosage of each examination and reducing costs. Thus, in view of the need to face alI steps of the radiographic process, and considering that QAP should aim also patient's satisfaction, from the clinicai standpoint, where the first issue is care, a methodology was worked out in such a way that it can be seen from an administrative standpoint (set up conditions to implement QAP; motivation and adoption of a working system aiming changes in values and attitudes; creating and bringing into practice flow control tables and tables for analysing film losses), and from a technical standpoint (set up of procedures to interpretate lost radiographies, and to get quality inspection and administration; data colIection regarding radiographic techniques, size of patient, dosage on surface entrance and quality of image; refresher course and set up of image criteria). FinalIy, it was considered the adoption of corrective measures for each step of the program, analysis of images before and after set up the image criteria, and establishment of economical advantages of the QAP in diagnostic radiology. On these basic, to stand out a reduction of 70% in the rejection of films, and a cost reduction of about 75% with an effective improvement in the quality of images were reached. The discontinuity of applying the QAP, implied an increase in the rejection of films and costs, returning to the preceding situation nineteen months later. Therefore, such programs should have the support of related authorities, not only because of its economical and technical aspects, but also because of the social ones.
