Resumo

A fotoestimulação intermitente (FEI) tem sido considerada um dos testes funcionais mais importantes nos exames neurofi siológicos clínicos por ser capaz de induzir, pelo mecanismo de ressonância fotorrecrutante, o surgimento de alterações patológicas não presentes no eletroencefalograma (EEG) espontâneo. Para detectar os efeitos fotorrecrutantes da FEI de 5, 8 e 12 Hz no EEG de 14 regiões cerebrais de 38 crianças e adolescentes normais de 3 a 17 anos, a técnica Teste F-Es pectral (TFE) foi usada para investigar a ausência de resposta. Para cada freqüência fixa de FEI os sinais EEG adquiridos em regiões corticais homólogas, tanto anterior quanto durante estimulação, foram subdivididos em M=10 segmentos de igual duração e, então, estimados os espectros e calculado o TFE. Esta técnica indicou a existência de harmônicos da FEI para os quais a hipótese nula de ausência de resposta pode ser rejeitada (a=0,05). O TFE evidenciou mudanças estatisti camente significativas em alguns harmônicos da freqüência de estimulação não detectáveis pelas análises temporal e espectral, normalmente empregadas. Tais achados sugerem o uso do TFE como técnica complementar aos métodos tradicio nais de detecção das respostas fotorrecrutantes, por exemplo na investigação da topografia do fotorrecrutamento e de sua composição espectral, em particular para as freqüências mais altas (sub-banda beta-2 e superiores) e nas áreas cerebrais anteriores onde a contribuição relativa de potência no EEG geralmente é mais baixa.

Abstract

In clinical neurophysiological examination, Intermittent Photic Sti mulation (IPS) is considered as one of the most important functional tests. It is capable to induce in the electroencephalogram (EEG) a manifestation of pathological alterations, which are not present in the spontaneous activity and occurs as a result of resonance-like photic driving mechanisms. The Spectral F Test (SFT) method was applied to detect photic driving effects (and hence for testing an absence of response hypothesis) of the IPS at 5, 8 and 12 Hz in the EEG of 14 brain regions of 38 normal children and adolescents (age range from 3 to 17 years). The EEG signals were acquired in homologous cor tical regions, before and during stimulation and then sectioned into M = 10 segments of equal duration. Next, for each fixed IPS frequency, the spectra were estimated and SFT was calculated. The results indi cated the existence of IPS harmonics for which the null hypothesis of lack of response (H0) could be rejected (a= 0.05). Moreover, some of these responses could not be observed in the spectral peak estimates. These findings suggest SFT to serve as an additional method for photic driving detection in topographic mapping more useful for low-amplitude responses (usually within the beta-2 sub-band and higher frequencies and in the anterior brain areas).
